If you have lost or had your military ID card stolen, you will need to provide certain documentation in order to receive a new one.
– Statement or document from Service
– Letter or memorandum from Soldier CDR/1SG
– Report from installation security office
– Report from installation security office
– CASDF 101, CASPC 201 or LES checklist signed by unit commander
If you are a dependent and your sponsor has lost their ID card, they must provide certain documentation in order to receive a new one.
– DD Form 1172-2 (for dependents of Active Duty, Guard and Reserve Soldiers)
– Power of Attorney (live with a local relative)
– Birth certificate / Citizenship ID card / passport
You may receive a new ID card after one of the following:
– MP report or statement/memorandum describing the date, place, and circumstances of the loss or theft
– Signed by the sponsor and a verifying official
– Court documents signed by Judge and filed
– Original or certified copy of SSN card
– Original or certified copy of birth certificate
– Certified filed marriage certificate
– Female military name change memo approved by MPD Chief
– Ward of the state court documents signed by Judge and filed
– Original or certified copy birth certificate
– Original or certified copy SSN card. Court document must state ward will be in the custody of sponsor for a minimum of 12 months
– Male sponsor must have court order that establish paternity & child’s original or certified copy birth certificate
– Paternity not established yet-letter from Social Security Administration stating Medicare started/purchased. Enrollment can occur as early as 60 days prior to 65th birthday
– College letter stating full time enrollment, working toward an associate degree or higher & anticipated graduation date. Sponsor must be present when issuing card or sign DD 1172-2
– Letter from an accredited college/university, signed by the school registrar stating full time enrollment, working toward an associate degree or higher & anticipated graduation date. Sponsor must be present when issuing card or sign DD 1172-2
– Tricare for life (medicaid/medicare) bring in the medicaid/medicare card for verification or if card is lost, letter from Social Security Administration stating Medicare started/purchased. Enrollment can occur as early as 60 days prior to 65th birthday.
– CASDF 101, CASPC 201 or LES checklist signed by unit commander
