Military members who are in the process of getting a name change will need to provide various documents to the Defense Enrollment Eligibility Reporting System (DEERS). These documents include: a valid state or federal government photo ID, original/certified copy of marriage certificate, Social Security Card, and Birth Certificate. The spouse must also provide a valid government photo ID and copy of marriage certificate. In addition to this, children must have a copy of their birth certificate and social security number. Furthermore, if the member has been approved to change their name through other means such as through a state or federal agency, a certification page will be required complete with a member signature. Lastly, dependents in the military who are 21-26 years of age and are not full-time students can be enrolled in the TRICARE Young Adult Program.
