You are authorized one (1) contracted shipment from your old
permanent duty station (PDS) to your new PDS.
You may also have one
Do-It-Yourself (DITY) Move at no extra shipment cost. (Counseling required prior
to movement of DITY Move)
No unaccompanied baggage shipment authorized
within CONUS.
If you ship more than one government contracted shipment
you may be charged excess cost. Weight allowance as prescribed by rank/pay grade
in Weight Allowance Table.
Member's responsibility to reimburse the
Government of any excess costs occasioned by this/these shipment(s). (i.e.,
attempted pickup and/or delivery, excess weight, more than one shipment, excess
mileage, unauthorized items/crating)
Packing and pickup dates as
determined by member subject to contractor
availability.
Ensure all personal property items are clean,
free of soil/pest infestation before pick-up date.
Temporary storage of
90 days is authorized. Storage beyond 90 days must be approved by the YPG &
MCAS Transportation Office.
Check inventory at origin. If a discrepancy
occurs, annotate items number in remarks block of inventory and exact
discrepancy, prior to releasing shipment to carrier.
Check DD Form 619
prepared by carrier at origin for complete accuracy of information and charges
recorded thereon.
Member/agent is responsible to sign inventory sheet(s)
and DD Form 619. Obtain copies of all paperwork prepared by the carrier for
movement of your property.
Member is responsible to contact origin and
destination Transportation office if there is any change in orders or there are
other factors that could affect delivery of the shipment.
Member is
responsible to contact destination Transportation Office immediately upon
arrival to give a point of contact for the ITO when property
arrives.
Member is responsible to request delivery from destination
Transportation Office or extension of storage period.
Member is
responsible to sign delivery documents and accept delivery at destination.
Member is responsible to gain prior approval for any extra pickup or
delivery from the applicable Transportation Office.
Designate an agent
to release property or accept property in absence of member and use of Power of
Attorney or informal letter of authority.
