All first-time and transfer students must provide the following
documentation:
1. Birth Certificate
2. Social Security Number
3. A
physical examination performed by a physician within a year of
enrollment. Requirements vary slightly with different school
systems.
4. A series of four DPT and three Polio Immunizations
5.
Immunizations against Measles, Mumps and Rubella - one after the first birthday
and the second prior to entry into kindergarten or into first grade, if the
child has not attended kindergarten.

Virginia
