General Requirements For All Applicants (New, Renewals and Duplicate Licenses and Instruction Permits)

Proof of Name, Birth Date, and if applicable, Sex Change
(e.g., Birth Certificate, Certified Marriage Certificate, Certified Decree of Name Change, State or Military I.D. Card, Alien/Refugee Registration I.D., Certified Physician's Certificate of Sex Change, etc.) Photocopies are not acceptable.
Note: For minor applicants between the ages of 15.5 through 17, a certified copy of the minor's birth certificate is required to establish proof of age and to determine who may sign the parental consent.

Proof of Social Security Number (e.g., Social Security Card, Previous Driver's License indicating Social Security Number, State or Military I.D. Card, etc.) Photocopies are not acceptable.

Number of Driver's Licenses
You must surrender all valid driver's licenses in your possession in order to obtain a State of Hawaii driver's license. No driver is permitted to hold more than one valid driver's license at any time.

Ineligibility

You will not be issued a driver's license, if:  your license has been suspended or revoked by a court of law;
you have failed to pass the required driver's examination;
you have not deposited proof of financial responsibility (such as an auto liability insurance policy, surety bond, etc.) if State law for this responsibility applies to you;
the examiner of drivers has good cause to believe that you would not be able to operate a motor vehicle safely upon the highways by reason of physical or mental disability;
you are under the age of 16 years; you are 16 through 17 years of age, and do not have parental consent; you have a traffic violation which is unsatisfied or is pending a court hearing; you do not have an Instruction Permit or a valid driver's license from another U.S. jurisdiction.
there is a stopper on the Problem Driver Pointer System (PDPS).

Special Requirements

Minor (ages 15 through 17) Applicants

Permit applications by minors must be signed by both parents or, when applicable, by the certified custodial parent, certified custodial guardian, or certified foster parents. (Only certified copies of court documents are acceptable.)

The spouse (who is an adult) of a minor applicant may sign the license application with proof of age and certified copy of the marriage certificate.

Minors who are legally emancipated may sign license applications upon presentation of the certified court document which granted emancipation.

New licenses will expire on your fourth birthday following the date of issue.

Effective January 1, 2001, the minimum age to acquire a permit is 15 and the minimum age to acqure a license is 16. Minors between the ages of 16 through 17 must pass and present certificates of completion of mandatory driver education and behind the wheel training from a DOT authorized instructor, before securing a Road Test.

Adult (ages 18 through 71) Applicants - New licenses and renewals will expire on your sixth birthday following the date of issue.

Senior (ages 72 or older) Applicants - New licenses and renewals will expire on your second birthday following the date of issue.

Instruction Permit Applicants

You may apply for a temporary instruction permit if, except for a lack on instruction in operating a motor vehicle, you would otherwise be qualified to obtain a driver's license.

You must obtain an Instruction Permit as a first step to qualify for a new driver's license.

The examiner of drivers shall test your: eyesight and determine if you possess the physical and mental capabilities to safely operate a motor vehicle; understanding of highway signs regulating, warning and directing traffic; knowledge of traffic laws, ordinances, or regulations of the State and City and County of Honolulu.

If the examiner of drivers is satisfied that you are qualified to receive an instruction permit, the examiner of drivers shall issue the permit to you. The permit is valid for a period of one year. Permits may NOT be renewed by mail.

Except when operating a motorcycle, or moped, you shall be accompanied by a person 18 years of age or older who is licensed to operate the category of motor vehicle being operated. The licensed person shall occupy a seat as near to you as is practical while the motor vehicle is being operated.

The temporary instruction permit does not allow you to operate a motorcycle or moped during the hours of darkness or carry any passenger.

Your temporary instruction permit for the operation of a motorcycle or moped may not be renewed nor shall you be issued another temporary instruction permit until you attempt the motorcycle skills exam.

Every person who drives any type of motorized vehicle is required to be tested and licensed. Please review the following information to assist you in applying and qualifying for a Hawaii Driver's License or for an Instruction Permit for passenger cars, motorcycles, motor scooters and mopeds.

All the driver licensing stations (Fort Street, Kaneohe, Wahiawa, Pearl City, Windward Mall, Kapolei and Waianae, plus the Commercial Driver License office at 99-501 Salt Lake Blvd.) have a new statewide computer/camera licensing system. With the new system, licenses are issued immediately rather than being mailed in six weeks.

Improvements provided by the new system are more accurately colored photos (no more "sunburned" look), electronic scanning of thumb print and bar coding of license numbers. The photo, thumb print, and signature are entered into a computer database - for future retrieval of information. The size of the license is not changed.

Effective January 1, 2001, the Social Security Number will no longer be printed on the plastic driver's license. A computer generated "alternate number" will take its place. However, the applicant must still provide the Social Security Number on the application form.

Beginning January 1, 2001, an applicant may indicate on the driver license application form if he/she has an advanced health care directive. If "yes" is indicated, the plastic license will indicate the letters "ahcd."

Effective May 7, 2001, the instruction permit for class 1, 2, 3 and 4 vehicles was extended to one year.

Effective January 2, 2002, all male applicants between the ages of 18 through 25, are required by Federal law, to be automatically registered with the United States Selective Service System upon signing the driver license application form for permits, license, renewals and duplicates.

Important Matters to Remember

The vehicle you intend to drive must have current license plates and registration card.

The vehicle you intend to drive must have a current safety check.

Your Hawaii motor vehicle insurance card must be current and valid. The name of the insured must be the same as the registered owner of the vehicle.

You must have all personal data proof documents available when reporting for a driver's license.

You should review traffic laws and rules of the road. You should study the Hawaii Driver's Manual prior to tests. (Manuals are available at local bookstores and State libraries.)

If you are under treatment for alcohol or substance abuse, you are required to have a medical clearance to obtain a driver's license.

If you are unable to read and/or write and understand the English language, provisions may be made for an oral examination (restrictions may apply).

You must have an Instruction Permit before you can apply for a Road Test.

If you are required to perform a road test, you may request a Road Test at the time of issuance of the Instruction Permit, or in person at any location for an appointment.

If you are 15 through 17 years of age and issued an Instruction Permit, you must hold the permit for a period of no fewer than 90 days. Also, you must be 16 through 17 years of age before being eligible to take the Road Test.

For Class 4 non-CDL applicants, a medical examination long form (DOT 4-766) may be required if the Division of Motor Vehicles does not have a valid form on file at the time of renewal or replacement.

VEHICLE REGISTRATION:

Non-resident active duty military personnel must submit a properly completed Non-Resident Certificate, Form DF-L (MVR) 50, for each registration, renewal or transfer or transaction.

*Form DF-L (MVR) 50 is available from your military personnel office or your military unit.


Out of State Permit - must have the following:

Current registration

Shipping document (Bill of Laden)

Fill out DF-L (MVR) 27 form (out of state permit application form)

HI safety Inspection
