California Law requires evidence of a health examination for
school prior to entry. It must be conducted by a medical professional licensed
in the United States. Report Forms can be obtained at the school office or at
the Riverside County Department of Health Services. A waiver (PM 171B) may be
signed if a parent does not want the child to have an examination.
SCHOOL
STAFF WILL REQUEST THE FOLLOWING ITEMS AT THE TIME OF REGISTRATION:

Evidence of birth date, such as a certified birth certificate, baptismal
certificate duly attested, or a passport.

. The Report of Health
Examination for School Entry must be signed and dated by the doctor
performing the physical. The physical exam may be done up to a maximum of six
months prior to entry into kindergarten.
. Proof of residency: (Two
Documents Required) Either a rent receipt, close of escrow papers, lease
agreement or mortgage verification, and a utility bill, tax bill, or bank
statement.
It is also recommended you provide the following information
at the time of registration to assist school staff with student
placement:

Immunization
Requirements
The following immunizations are required for
school enrollment by the State of California. An official record of the
student's immunization record must be presented as verification of immunization.
This record can be a California Yellow Immunization Card, a medical record from
the physician or an official school record. The record must be signed or stamped
by the physician or by a school official.
- TB All students enrolling in a
Riverside County school for the first time (including all kindergartners) must
have an intradermal (Mantoux) TB skin test given within the last 15 months.
Written evidence of a negative test must be submitted prior to enrollment. (If
students have previously attended a Riverside County School and have not been
out of the country for more than a year, they do not need a TB test)

four (4) day grace period is allowed if vaccine was given 4 days prior to
birthday.
All immunizations must be current, meaning students have
completed the above requirements or are not yet due for another dose.
Students shall be exempt from immunization only if a parent or guardian
signs a waiver after speaking with the school health technician or school nurse.
Parents must also provide either a written statement on the waiver indicating
that immunization is contrary to his or her beliefs. OR a written statement from
a licensed physician indicating the student's physical condition or medical
circumstances are such that immunization is not considered safe.

March ARB.
The Riverside Unified School District enrolls 42,012
students in kindergarten through grade 12, and covers a major portion of the
city of Riverside, as well as the Highgrove and Woodcrest areas outside the
city. The District operates 5 comprehensive high (grades 9-12)school, 2
continuation high schools, 1 alternative school, and 1 opportunity high
school. 6,714 students (16%) are classified as Limited English
Proficient, and 13,567 (32.3%) qualify for the compensatory education program.
The district employs 1,830 classroom teachers, and 127 staff members for pupil
services. All comprehensive high schools are accredited by the Western
Association of Schools and Colleges. Forty-two percent of the teaching staff has
master's degrees and several hold doctorates. Programs & services for
special needs students include modification of the regular classroom, provision
of related services and staff such as speech therapy, adapted PE, resource
specialist, itinerant teacher (hearing- & visually-impaired students), and
self-contained classroom.
Alvord Unified School District serves the
western portion of the City of Riverside and a small portion of eastern
Corona. The district has an enrollment of 19,441 students, and
employs 860 classroom teachers and 58 staff for pupil services. 7,652 students
(39.4%) are classified as Limited English Proficient, and 8,536 (43.9%) qualify
for the compensatory education program. The district operates two comprehensive
high schools and one alternative/continuation high school. Programs
and services are available to support special needs students. Other district
programs include Gifted Education (GATE), after-school programs (Prime Time,
middle school level), and P.E.P. grant programs (Physical Fitness, all grade
levels).
The Jurupa Unified School District operates 2 comprehensive,
2 continuation high schools, 1 community day school, and an Adult Education
Center. The district has an enrollment of 20,924 students, and employs 912
classroom teachers and 60 staff for pupil services. 5,967 students (28.5%) are
classified as Limited English Proficient, and 10,799 (51.6%) qualify for the
compensatory education program. The district includes the communities of
Riverside & Mira Loma. A library is available in each of the
schools
The Moreno Valley Unified School District operates 4 high, 1
K-12 charter, 1 alternative, 1 continuation, 1 community day
school. The District enrollment is 34,792 students. 10,087 students
(29%) are classified as Limited English Proficient, and 25,734 students (74%)
qualify for the compensatory education program. The district employs 1,552
classroom teachers, and 110 staff members for pupil services. A variety of
programs, services, and personnel are provided for Limited English Speaking
(LEP), Gifted & Talented, and Special Needs students. Support services for
all students, as needed, include speech/language, tutoring and
counseling.
The Val Verde Unified School District operates 2 high
schools and 1 continuation school. The district enrollment stands
at 132,447 students. 3,771 students (28%) are classified as Limited
English Proficient, and 6,713 students (49.9%) qualify for the compensatory
education program. The district employs 547 classroom teachers, and 36 staff
members for pupil services. A variety of programs, services, and personnel are
provided for Limited English Speaking (LEP), Gifted & Talented, and Special
Needs students. Support services for all students, as needed, include
speech/language, tutoring and counseling. Extra-curricular activities are
offered at all grade levels, and some of the elementary schools offer an
after-school child care, recreation & enrichment program, either through
grant funding or a cooperative arrangement with the city parks & recreation
department.
The Corona-Norco Unified School District operates 4 high,
4 continuation, 1 alternative, 1 special education, and 1 community day
schools. The district has 43,998 students. 7,730 students (16.8%) are
classified as Limited English Proficient, and 8,321 students (18.9%) qualify for
the compensatory education program. The district employs 2,079 classroom
teachers, and 121 staff members for pupil services. A variety of programs,
services, and personnel are provided for Limited English Speaking (LEP), Gifted
& Talented, and Special Needs students. Support services for all students,
as needed, include speech/language, tutoring and counseling.
The
Hemet Unified School District operates 2 high, 1 K-12, 1 alternative, 1
continuation, and 1 community day schools. The district has 19,693
students. 2,577 students (13.1%) are classified as Limited English Proficient,
and 12,376 students (62.8%) qualify for the compensatory education program. The
district employs 908 classroom teachers, and 63 staff members for pupil

The Temecula Valley Unified School District
operates 2 high schools, 1 K-12, 2 alternative, and 1 continuation

The Murrietta Valley Unified School District(MVUSD)has 2 high
schools, 1 alternative and 1 continuation school. The district
enrollment is 17,480 students. 432 students (2.5%) are classified as Limited
English Proficient, and 405 (2.3%) qualify for the compensatory education
program. The district employs 799 classroom teachers, and 57 staff members for
pupil services. A variety of programs, services, and personnel are provided for
Limited English Speaking (LEP), Gifted & Talented, and Special Needs

The Perris Union High School District operates 7
schools (1 middle, 3 high, 1 alternative, 1 continuation, 1 community day),
which serve 7,498 students. 770 students (10.3%) are classified as Limited
English Proficient, and 4,701 (62.7%) qualify for the compensatory education
program. The district employs 292 classroom teachers, and 35 staff members for
