If you had a child born recently or adopted a child, the
following actions must be completed by you as soon as possible.
MPF
CUSTOMER SERVICE ACTIONS:
A. DEERS (Defense Enrollment Eligibility Report
System) Enrollment - DD Form 1172, (to accomplish this action you must have a
copy of the birth certificate or certificate of live birth from the state or
from the hospital the child was born) B. Update Emergency Data Form
(DD Form 93), Update of PCIII C. SGLI changes, if
applicable. This can all be accomplished in Bldg
1255.
ACCOUNTING AND FINANCE (AFO) ACTIONS:
A. Take
your child's birth certificate to the local AFO B. BAQ-AF Form 594
(if applicable) C. New withholding statement (W-4). Finance Office is
located in Bldg 1255.
LRAFB CLINIC:
A. Initiate medical
records for your newborn child/children B. Initiate
medical care card (if applicable) C. TriCare
These steps will
save you confusion and inconvenience if followed promptly and
properly. You may want to visit the legal office to prepare or change
a will as a result of a child's birth.
If your spouse is in the military
or if you are single and this is your first child, you now fall under the
Dependent Care Program and you must report to your Unit Orderly Room for
dependent care counseling and preparation of AF Form 357. These
actions are required by AFI 36-2908, Dependent Care
Responsibilities.
Programs available to assist you in healthy
parenting:
The mission of the Family Advocacy Program (FAP) is to prevent
the occurrence and reduce the severity of family maltreatment. Our goals are to
provide education and treatment programs to reduce family conflict, and to
strengthen family cohesion. The FAP recognizes that the family plays an integral
role in supporting Air Force readiness by having a direct impact on the active
duty member's capacity to perform assigned duties. Family Advocacy has a variety
of programs that promote strong, healthy families by providing support,
prevention, and treatment services to eliminate the risk of all forms of family
maltreatment.
The Family Advocay Office presents a variety of programs
through the New Parent Support Program. The New Parent Support
Program offers many supportive and educational services to all expectant parents
and parents with children up to age three. This is a worldwide Air Force program
available at no cost to families. The New Parent Support Program objectives are
strengthening families, mission readiness, and community resiliency. Once
entering the program, participants receive the monthly Parent Connection
newsletter by email and may register for classes, workshops and groups. The
"What To Expect" series of books for pregnancy, first year, and toddlers are
provided to all families in the program. Home visitation by a maternal/child
nurse provides comprehensive education on maintaining a healthy pregnancy,
newborn care and parenting. Family wellness counseling tailored to fit your
family's needs is provided by a licensed social worker. This may include
marital, family, parent, individual, and child counseling. Classes, workshops,
