Defense Enrollment Eligibility Reporting System
(DEERS). DEERS is a computerized database of military sponsors,
families, and others worldwide who are entitled under the law to TRICARE
benefits. DEERS registration is required for TRICARE
eligibility. All uniformed service sponsors (active, reserve or
retired) should ensure that their family status (marriage, death, divorce, and
new child) and residential address are current in DEERS at all
times.
Registration in DEERS: Active duty and retired service
members are automatically registered in DEERS, but they must take action to
register their family members and ensure they're correctly entered into the
database. Mistakes in the DEERS database can cause problems with
TRICARE claims, so it is critical to maintain your DEERS
information.
Verifying and Updating DEERS Information: You can
verify your DEERS Information by contacting your regional TRICARE managed care
support contractor, you local TRICARE Service Center or Personnel Support
Detachment The local TRICARE Service Center is located in Naval Hospital
Lemoore. Sponsors or registered family members may make address
changes, however, only the sponsor can add or delete a family member from DEERS,
and proper documents are required such as a marriage certificate, divorce
decreee and/or birth certificates. To update your DEERS
information: visit the Personnel Support Detachment; or contact the
Defense Manapower Data Center Support Office (DSO) at 1-800-538-9552; or FAX
address changes to DEERS at 1-831-655-8317; or, mail the address change to the
Defense Manpower Data Center Support Office, ATTN: COA, 400 Gigling Road,
Seaside, CA 93955-6771; or, go online to update your
information.
Medical Aspects of TRICARE Program. TRICARE is
the Department of Defense's worldwide health care program for active duty and
retired uniformed service members and their families. TRICARE
consists of TRICARE Prime, a managed care option; TRICARE Extra, a preferred
provider option; and TRICARE Standard, a fee-for-service
option. TRICARE For Life is also available for Medicare-eligible
beneficiaries age 65 and over. For more information about TRICARE

All appointments for the Naval Hospital Lemoore are made for TRICARE
Prime enrolles through Central Appointments at 559-998-0889. All
other eligible beneficiaries will be treated on a space available
basis.
Active duty sick call is a located on the Operations Side Branch
Clinic opened Monday through Friday.
PROCEDURES FOR ACTIVE
DUTY AND TRICARE PRIME ENROLLEES AFTER DUTY HOURS CARE: For true
EMERGENCIES (where there is a danager to life, limb, or sight) - Call 911 or go
to local Emergency Room hospital. CALL YOUR regional TRICARE office
within 24 hours after the visit.
All other care MUST be
pre-authorized by YOUR Primary Care Manager (PCM).
