Head Start programs partner with community agencies &
organizations in the course of implementing the Head Start
services.
Eligibility for Early/Head Start is based on the federal
poverty income guidelines which are updated each year by the Census
Bureau.
In order to register your child, you must have the following
documents:
Proof of household income within the past 30
days.
Check stubs or employer verification by letter
TANF/AFDC
Card
Social Security Award
Letter
Identification.
Child's Birth Certificate
Social
Security Card (Child & Parent)
Current Medical
Records.
Current physical and health history (include doctor's
signature)
DHS medical card
Immunization Records and TB, Hemoglobin and
Lead
Dental Exam Form (include dentist's signature)
Three
complete local emergency contacts
Fees are govern by each individual
community program. The program closest to where you live.
