The Naval Health Clinic Great Lakes(NHCGL) and the North Chicago VA Medical Center (NCVAMC)now share in providing healthcare services. Great Lakes is the only Military Treatment Facility (MTF) in the Chicago area and in the Midwest. Its primary mission is to provide clinical inpatient and outpatient services for active duty Navy personnel and members of the uniformed services.  

NOTE:  If the MTF still has Primary Care capacity after enrollment of all eligible members of priority groups 1 through 3, non-enrolled beneficiaries (groups 4 and 5) may continue to access health care at the MTF, on a space-available basis.

Currently the NHCGL has 4 branch clinics and 20 clinics at:

1.  Recruit Training Command, Great Lakes (buildings 1007 & 1523).
2.  Training Support Command, Great Lakes (building 237).
3.  Branch Medical Clinic, Defense Finance Accounting Service, Cleveland Center, Cleveland, OH.
4.  Industrial Hygiene, Branch Clinic, Crane, IN.

All outpatient services will continue at NHCGL building 200H.  ALL NON-URGENT appointments can be made by calling (800) 941-4501.

There is NO EMERGENCY ROOM at the Naval Health Clinic Great Lakes (NHCGL). For emergency care, go to North Chicago VA Medical Center(NCVAMC) located on south east corner of Green Bay Road and Buckley Road in North Chicago.

On base emergency services (Fire, ambulance, Police) are accessed by dialing extension 3333.

NCVAMC Emergency Department will be open 24 hours a day for emergency care.

Poison control is accessible by dialing 1-800-942-5969.

Customer Relations can be reached at (847) 688-5929.

In patient medical and surgical services are also provided at NCVAMC.

NCVAMC is designated as a TRICARE network provider.  Co-pays apply as to all Retirees and Non-Prime beneficiaries.  Get more information on your TRICARE benefits, by visiting their website.

NCVAMC pharmacy will only provide medications associated with an Emergency Department visit or discharge from inpatient services. NCVAMC will NOT provide refills or routine medications.

To avoid costs assoicated with using network pharmacy, call NHCGL pharmacy for a refill when 75% of current medication has been completed. Pharmacy refill number is (800) 552-8569.

NCVAMC requires the patient's social security number and ID card to provide services for the following: Active Duty, family members, and prime eligible enrollees.

Surgeries within the VA will be perfomed by Navy surgeons in a new Operating Suite.

Inpatient care will occur within a new ward built to accommodate the additional patient census.

For school physicals contact the TRICARE Service Center for appointments.

PLEASE BRING THE FOLLOWING ITEMS:

*  IMMUNIZATION RECORD
*  HEALTH RECORD (If a record is not maintained at the hospital)
*  SCHOOL PHYSICAL FORMS, IF AVAILABLE
