Dental Care-Active Duty Soldiers: Patients with dental
emergencies report to the Hospital Emergency Service for after hours
care. Severe pain, swelling, or trauma are considered
emergencies. After hours care is limited to emergency treatment
only.
Dental Care-Family Members: CONCORDIA = Family members receive
dental care from civilian dentists using the TRICARE Active Duty Family Dental
Plan (ADFMDP) = CONCORDIA. ADFMDP offers comprehensive insured dental
care for family members of active duty soldiers. Families are highly
encouraged to establish themselves with a civilian dental practice soon after
arrival to Fort Polk. Emergency care for family members is provided
through their civilian dentist; however in the case of severe pain and/or trauma
they may be taken to the E.R. of Bayne Jones Hospital. An Active Duty
Dentist will be notified to give emergency treatment until the civilian provider
is notified. Provider Lists may be obtained from your Hospital Health
Benefits Advisor by calling (318)531-3974/3908. The newly arriving
Service and Family Members will have an opportunity to talk face to
face with the Benefits Advisor when they attend Newcomer Orientation held at
ACS, Bldg. #920, Bell Richard Ave., every Thursday.
