NEW STUDENT ENROLLMENT
New students
should bring the following:
* Last grade
card
* A transfer sheet (if provided)
* A shot
record along with any other information available from previous
school
* A current ICP and diagnostic report, if special
education
SCHOOL AND SPORTS PHYSICALS
Students who are
entering the Waynesville District for the first time must have a recent physical
examination prior to entrance into school. A physical examination
must have been completed by a duly licensed medical practitioner within the last
twelve months. Physical exam forms are available at the Fort Leonard
Wood General Army Hospital, local doctors' offices, and individual
schools.
If your student will be playing sports during the upcoming
school year, he/she must have a physical dated after 1 February of the current
school year on file at the athletic office before the first day of
practice. Students playing sports will use the same physical forms as
used by students entering school for the first
time.
IMMUNIZATIONS
Missouri state law requires that all
immunizations must be current and up to date before attending
school. No student will be allowed to attend school if an
immunization is past due. New students must present immunization
records that comply with Missouri state law requirements at the time of
registration. Missouri law requires all kindergarten through eleventh
grade students obtain Hepatitis B immunizations. This requires a
series of three shots which must be completed or in progress when school

STUDENT MEDICATIONS
Students wishing to
take any medications (prescriptions or over-the-counter) at school must have a
new medication authorization form on file for the upcoming school
year. All medications must be in a properly labeled bottle
and kept at the nurse's office. Students are not allowed to take any
medications (to include Tylenol or cough drops) at school without the proper
paperwork being on file at the nurse's office. Students are not
allowed to carry any medications with them at school or to and from school
except an inhaler. The student may carry the inhaler with them only
if a signed release form from their physician and parent are on file at the
nurse's office. It is also requested that an emergency asthma action
plan be on file and updated each school year.
