1. Department of Defense policy is to utilize Army dental facilities to provide dental care and promote dental health for uniformed service members in support of military readiness. Active duty family member care, to include dental emergencies, should be handled by their private dentist and is provided through enrollment in the Family Member Dental Plan (FMDP), currently sponsored by United Concordia. Active duty soldiers should enroll their spouse and children in the program by filling out the enrollment application at their unit. Information regarding the names and phone numbers of local dentists who accept military family members in the program is available at all Fort Jackson dental clinics.

2. Family members who are not eligible to be enrolled in the FMDP are seen on a space available standby basis. This includes family members of active duty deceased, as well as family members having sponsors with less than 24 months before ETS who are not eligible for FMDP.

3. Department of Defense policy restricts the Dental Activity (DENTAC) to offering only emergency care for retirees, retired family members, and active duty family members eligible to be enrolled in the FMDP. A valid dental emergency is defined as significant pain, infection or trauma. Emergency dental care is provided at all the Fort Jackson dental clinics. The dental clinics operate regularly between 0715-1215 and 1315-1615, Monday through Friday, and emergency care is available preferably during sick-call hours, but also during normal duty hours. After duty hours, this type of care is provided in the Moncrief Army Community Hospital Dental Clinic, after the patient has processed in at the hospital Emergency Room.

4. The dental clinics at Fort Jackson are:     Caldwell Dental Clinic (Bldg 4590, Strom Thurmond Blvd, 751-7025/5178;  Hagen Dental Clinic (Bldg 5330, Moseby St.; 751-3255/4091;  Department of Dentistry (Moncrief Army Hospital, 751-5908/2241;  Physical Exam Station (Bldg 1890, Washington Road, 751-3105/3106).        Active duty personnel should consult unit members to determine which clinic services the unit, and call or come in and an appointment will be made.
