There is no dental care provided at the Ainsworth Clinic for
active duty military. The military person seeking care must go to the
Ainsworth Clinic and request a form to bring to the Veterans Affairs
Hospital. The VA provides dental care to active duty
military. For family members, please seek a provider who accepts your
dental/health coverage.

DENTAL
CARE: The Primary Provider for dental care for all active duty
personnel assigned to the greater NYC area and Long Island is the VA hospital
Dental Clinic adjacent to Fort Hamilton (718) 630-3651. This is the
only VA clinic available to the military.
All care at the VA Dental
Clinic is coordinated through the DOD Liaison Office on the 4th floor of the
hospital (718) 630-2833. All personnel must first report to the DOD
office before treatment.
The Army point of contact and authority for
oversight of care for this area is the Clinic Commander, U.S. Army Dental
Command, Fort Monmouth, New Jersey: DSN 992-7060 or Comm (908)
532-7060.
Any unresolved questions pertaining to care at the VA Clinic or
other clinics available to soldiers may be directed to Fort Monmouth.
It
is recommended that all active duty family members enroll in the TriCare Active
Duty Family Member Dental Plan. All TriCare insurance inquiries
pertaining to eligibility, location of participating dentists, billing, etc. may
be directed to 1-800-866-8499 or (718)630-4611.
