U.S. ARMY DENTAL ACTIVITY (DENTAC)
Complete dental care is
available for all active duty soldiers. Fort Carson has
state-of-the-art dental facilities. Commanders and First Sergeants
should be able to direct you to the correct facility. Due to limited
resources and the emphasis on active duty dental readiness, unfortunately, there
is no space available at present for other than active duty
patients.
AFTER HOURS EMERGENCY DENTAL CARE
After hours dental
emergencies (bleeding, swelling, trauma) should report to the Evans Army
Community Hospital Emergency Room. The DENTAC staff is committed to
providing you with the best care available and will address any questions or
concerns you may have. If, for some reason, you are not satisfied
with their response, you may address your concerns to DENTAC Headquarters at
719-526-2006.
FAMILY MEMBERS OF ACTIVE DUTY SOLDIERS
Family
members of active duty soldiers may apply to be enrolled in the TRICARE Dental
Program (TDP) operated by United Concordia. The TDP is a high
quality, cost-effective dental care benefit for family members of all active
duty Uniformed Service personnel and to Selected Reserve and Individual Ready
Reserve (IRR) members and/or their families. The TDP has cost shares,
benefit maximums and coverage limitations similar to those that employees of the
Fortune 500 companies receive. The only requirement is a one-year
service commitment.
Covered Services
-- Diagnostic/Preventive, Basic Restorative/Sealants, Endodontics,
Periodontics, Oral Surgery, Other Restorative/Prostodontics/Orthodontics,
General Anesthesia and IV Sedation.
Enrollment
-- Enrollment forms may be downloaded at the United Concordia website found in
Contacts/Links, or may be obtained by visiting a local treatment facility or
Evans Army Community Hospital Patient Representative Office. The
first month premium must accompany the initial enrollment. Subsequent
premiums are deducted from pay as an allotment. As long as the
Enrollment/Change Form and first month premium are received by the 20th of the
month, coverage will begin after the 1st of the following
month.
Cost Shares -- Cost shares
apply for services other than preventive and emergency services performed by a
Participating Dentist. Refer to the benefits booklet for limitations,
restrictions, and cost shares.
Participating
Dentists -- A list of participating dentists can be located at the United
Concordia website.
Coverage is worldwide and portable anywhere you
visit. United Concordia will coordinate benefits with other insurance
companies if you have other dental insurance. United Concordia
Company has done a terrific job in administering the TDP for the Department of
