Enrollment in the Exceptional Family Member Program is mandatory
for all Navy personnel who have family members with ongoing medical,
psychological and/or special education needs. For more information, please
contact the Information & Referral Specialist at the Fleet & Family
Support Centers, EFMP Liaison or the EFMP Coordinators located at the
local branch medical centers.
Supplemental Security Income (SSI):
If
your are receiving SSI or other SSA sponsored assistance, you must contact them
before transferring to see if SSI benefits are transferable.. The Social
Security Administration has answers to questions about Supplemental Security
Income (SSI), Social Security Disability Insurance, and other monetary
assistance programs.
Program for Persons with Disabilities:
Program
for Persons with Disabilities provides financial assistance to qualifying active
duty family members who need specialized care, training, and / or equipment that
may not be available or may be inadequate through public resources. In order to
qualify for Program for Persons with Disabilities (PFPWD) the beneficiary must
have a serious physical handicap or must be moderately or severely mentally
impaired. The Special Program Coordinator will coordinate all of the PFPWD
services for beneficiaries that participate in the program to assure that the
beneficiary's health care needs under PFPWD are met. If you would like to apply
for this program, you must first complete the Medical Management Referral Form
and fax it to the number at the top left of the form. A Special
Program Coordinator will contact you and can assist you with completing the
application forms in the PFPWD Packet. The Medical Management Referral Form can
be downloaded from Health Net's website listed in Contacts.
